Consent and Cancellation

Welcome to Jen Bennett Sports Massage and Personal Training.  


A health screening questionnaire and consent form must be completed and signed before sports massage or personal training can commence.

 

Sports Massage.

Jenny Bennett is as fully qualified and insured sports massage practitoner.

The physical response to soft tissue work varies and cannot be predicted as every individual is different. There is no guarantee that the treatment given will help the condition you are seeking treatment for and there is a risk that any treatment will cause some discomfort or aggrevation of the existing condition in the short term.  Every effort will be made to minimise these risks by evaluation of prelimary information relating to your health and fitness and by seeking feedback to each different element of treatment as it is introduced.

The treatment given will vary depending on the indvidual needs of the client and may include any of the following techniques: maintenance massage, deep tissue massage, crossfiber massage, trigger point massage, myofascial massage, muscle every techniques and neuromuscular techniques.  You may find some of these techniques uncomforable; the intensity of the techniques can be altered to suityour comfort level so please let the therapist know if at any timeyou wish the intensity to change or the treatment to stop.  Should you prefer not to receive any of a particular kind of treatment, please let the therapist know before commencement of the massage or at any time during the massage.  Sports massage involves massaging different parts of the body as relevant to the client's needs and wishes, excluding contraindicated areas.  If at any time the client is uncomfortable with the massage, please inform the therapist and the massage will be discontinued.

 

Personal Training.

Jenny Bennett is a fully qualified and insured personal trainer.

Before we begin, I would like to inform you about the purpose, risks and benefits of my personal training programs.  This will help you to understand more about the personal training as we run through these details including your rights and responsibilities in order for you to reap the full benefits of the program.

There are chances that during the period of workout, adverse changes might occur such as dizziness or fainting.  In rare instances, it may lead to a heart attack, stroke which may be a cause of death.  Sometimes, injuries during participation of the exercise programs might happen that may involve the joints, ligaments, muscles and tendons. It shall be your responsibility to ensure the prevention of these during the workout.  You do not need to push hard if your vody canot make it through yet.  Take time and let the progress come in time, gradually.

Your privacy is important.  All information I obtain from you before, during and after the fitness training program shall be kept confidential and will not be released to anyone without your prior consent.

 

Cancellation Policy

All clients must provide a minimum of 24 hours notice if they need to reschedule or cancel. Failure to do so will result in the full amount of the session being charged.

No shows will be charged in full.

Exceptions will be considered for unforeseen circumstances.

If the trainer is unable to conduct a session, the session will be rescheduled at a mutually convenient time.
